1. Complete all items requested on the form, making sure to provide a correct email address and telephone number.
2. Enter the code (exactly as shown) in the box provided.
3. Click "Submit" to email your "Session Request Form" to Carol. (A window will pop up confirming your email has been sent.)
4. Next, please click the "Pay For Your Session" Buy Now button.
5. You will be directed to the PayPal site where you can either log into your PayPal account or sign in as a guest to pay for your session.
6. Once your payment has been processed, you will be sent an email confirming that your payment has been received.
7. Within 24 hours of payment confirmation, you will be contacted via the telephone number you provided to schedule your session at a time that is mutually convenient. (In the event your payment is not received within 24 hours of receipt of your Session Request Form, your request will be considered void and you will need to complete a new session request form.)
   
   In the event no mutual time can be scheduled, your payment will be refunded in full within 72 hours.
8. In the event that you need to reschedule your appointment, please contact Carol via email at least 24 hours prior to your appointment time, notifying her of your intent to reschedule.
   
   If you do not provide written, email notification to Carol of your intent to reschedule at least 24 hours prior to your scheduled session, the entirety of your payment of $125 will not be refunded as there is a 24-hour cancellation policy in effect for all booked sessions.
9. To cancel your appointment and receive a full refund, please contact Carol via email at least 24 hours prior to your appointment time, notifying her of your intent to cancel. (Your payment of $125 will be fully refunded within 72 hours.)
   
   If you do not provide written, email notification to Carol of your intent to cancel at least 24 hours prior to your scheduled session, the entirety of your payment of $125 will not be refunded as there is a 24-hour cancellation policy in effect for all booked sessions.
